1. Team Registration Form – use this form to reserve your team’s spot in the tournament
2. Waiver/Roster Form – bring this completed form to Registration Tent 1-hour before your first game to Check-In and receive “Player Packs” for each player
3. Player Replacement Form – bring this form to Check-In at the event, if you had to add, subtract or change players from your original registration
4. Directions to Frisco Square – print this form for directions, parking instructions, and nearby restaurants and attractions
5. Court Layout – print this form to view layout of the courts and tournament site
